About the Role

This is an exciting opportunity to join our team as a Minor Injury Practitioner - Community Specialist. As a key member of our community healthcare team, you will be responsible for providing specialist assessment and treatment to patients in the community setting.

Key Responsibilities

  • Provide specialist assessment and treatment to patients in the community setting, working closely with multidisciplinary teams to ensure comprehensive care.
  • Develop and implement treatment plans that meet the individual needs of patients, taking into account their medical history, social circumstances, and personal preferences.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Conduct regular reviews and assessments to monitor patient progress and adjust treatment plans as needed.
  • Provide education and support to patients and their families to promote self-management and independence.

Requirements

  • Previous experience as a Band 6 Practitioner in the community, with a strong background in minor injury care.
  • Proven ability to work effectively in a community setting, with a focus on patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with multidisciplinary teams.
  • Strong leadership and management skills, with experience of providing supervision and training to staff.
  • Ability to work autonomously, with a high degree of independence and initiative.

Desirable Criteria

  • Experience of budget management and financial planning.
  • Leadership training or willingness to complete.
