We are seeking a highly skilled Clinical Pharmacist to join our team at Green Cross Group Practice. As a Clinical Pharmacist, you will play a vital role in supporting the delivery of high-quality patient care and contributing to the development of our pharmacy services.

Key Responsibilities:
  • Conduct structured medication reviews with patients and make adjustments to their medication regimens as needed.
  • Manage the repeat prescription system and handle acute prescription requests in a timely and efficient manner.
  • Reconcile medicines following discharge from hospitals, intermediate care, and care homes, and work with patients and community pharmacists to ensure continuity of care.
  • Develop and implement systems to ensure safe and effective medication management for high-risk patients.
  • Provide expert clinical medicines advice to patients, healthcare professionals, and other stakeholders.
Requirements:
  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Minimum of 2 years' experience as a pharmacist in the NHS or community pharmacy.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, healthcare professionals, and other stakeholders.
