Job Title: Senior Mental Health Practitioner

This is an exciting opportunity to join our team as a Senior Mental Health Practitioner, working within the Primary Care Network (PCN) to support children and young people who are experiencing mental health difficulties.

About the Role

As a Senior Mental Health Practitioner, you will be responsible for providing interventions, advice, consultation, and supervision to colleagues and external partner agencies in line with THRIVE and clinical competency.

You will work closely with the team to promote the Thrive model of service delivery and develop innovative and creative practice.

Key Responsibilities
  • Provide direct therapeutic interventions, including assessments, formulation, and therapeutic input into children, young people, and families referred to the service.
  • Undertake risk assessments and care plans in line with the THRIVE framework.
  • Work sensitively with children and young people from diverse backgrounds.
  • Screen referrals to children and young people's services.
  • Participate in the team's duty rotas.
  • Deliver care coordination, ensuring effective assessment, care planning, and risk management processes.
  • Produce clinical formulations and intervention plans based on evidence-based practice.
  • Work collaboratively with other team members to enhance and develop services for young people and their families.
  • Identify and respond to child protection issues in accordance with Local Safeguarding Children's Board and Northwest Boroughs Healthcare NHS Foundation Trust policies.
  • Maintain accurate and comprehensive electronic clinical records.
  • Engage with routine outcome measures and input data into clinical records.
  • Work collaboratively with children, young people, and their families to contribute to the trust's service user participation agenda.
Requirements
  • Registered Core Profession Social Work or Nurse with up-to-date professional registration.
  • Postgraduate or equivalent level of experience.
  • Experience of mental health assessments and therapeutic interventions with children and young people.
  • Ability to communicate effectively with clients, families, carers, and professionals.
  • Knowledge and skills in approaches within the field to meet patient needs.
  • Level of knowledge of mental health needs of children and adolescents.
  • Ability to work in culturally diverse communities and take full clinical responsibility for patient care.
