Job Overview

An exciting opportunity has arisen to join our team as a Clinical Lead Practitioner, where you will play a key role in providing clinical leadership and support to our clinical staff. You will work closely with our Pennine Care NHS Foundation Trust colleagues and external partners to identify and support patients who are struggling with their mental health in the community.

This is an interesting and inspiring time to join our neighbourhood mental health team as we are developing a service for the residents of Tameside to access support for their mental health at a time and place that is right for them.

Main Duties and Responsibilities
  • Provide clinical leadership, support, and supervision to clinical staff.
  • Support an identified team of practitioners with advice and guidance on the management of clinical cases and hold a small caseload of patients in the 12-week pathway.
  • Work as an integral part of the Neighbourhood Mental Health Team (NMHT) and participate in extended working hours as required by the needs of the service.
  • Provide clinical leadership and structured clinical supervision to the team, ensuring consistency and quality of care for all service users under the care of the NMHT.
  • Support safe, efficient, and effective high-quality transitions in and out of the NMHT and ensure the team works in a collaborative manner within the NMHT/LLW with other PCFT services, primary care, and other partners.

Detailed Job Description and Main Responsibilities

Post: Clinical Lead Practitioner
Band: Band 7
Location/Base: Neighbourhood Mental Health Team/Living Well
Responsible to: Operational Team Manager
Main Contacts: Internal or external contacts

Job Summary:
• Provide clinical leadership, support, and supervision to clinical staff.
• Support an identified team of practitioners with advice and guidance on the management of clinical cases and hold a small caseload of patients in the 12-week pathway.
• Work as an integral part of the NMHT and participate in extended working hours as required by the needs of the service.

Main Duties and Responsibilities:
• Provide clinical leadership and structured clinical supervision to the team, ensuring consistency and quality of care for all service users under the care of the NMHT.
• Support safe, efficient, and effective high-quality transitions in and out of the NMHT and ensure the team works in a collaborative manner within the NMHT/LLW with other PCFT services, primary care, and other partners.
• Liaise closely with social care leads, housing associations, IPS workers, and other agencies to promote social support and aid recovery and promote this way of working within the team.

Managerial Responsibilities:
• Provide case management supervision to the staff team.
• Analyze, validate, and minimize any internal pathway blockages.
• Be an effective decision-making and clinical resource to support the Team manager in the selections and recruitment of staff, and develop and promote strategies aimed at staff retention.

Clinical Responsibilities:
• Provide guidance and support to clinical staff in the management of cases and work within the 12-week pathway.
• Be responsible for the development and maintenance of effective clinical pathways between NMHT and other services.

Education and Training:
• Organize the training of students, ensuring they are provided with the necessary experiences to develop their skills by allocating them an experienced supervisor who will provide assessment, supervision, and support during their allocation to the CMHT.

Administration and Information Technology:
• Ensure accurate, up-to-date records of visits and observations for people receiving services from NMHT and comply with team handover policies, in line with the Trust's standards for record keeping.

Professional Responsibilities:
• Practice at all times in accordance with the Professional Codes of Conduct/Practice as set out by the relevant professional body.

Clinical Governance:
• Lead and develop Clinical Governance initiatives within the team.
