Job Summary

We are seeking a highly skilled and compassionate Home Health Registered Nurse to join our team at AdventHealth Manchester Home Health. As a key member of our care team, you will be responsible for coordinating and directing the care of our patients in their homes, ensuring they receive the highest quality of care and support.

Key Responsibilities
  • Assess and Evaluate Patient Needs: Conduct comprehensive assessments of patients to identify their physical, functional, psychosocial, and educational needs.
  • Develop and Implement Care Plans: Create individualized care plans in consultation with patients, families, and healthcare providers to address patient needs and promote independence.
  • Provide Skilled Nursing Care: Deliver high-quality nursing care to patients in their homes, including preventative and rehabilitative procedures, and prescribed treatments.
  • Coordinate Care Services: Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Monitor and Evaluate Patient Progress: Regularly evaluate patient progress, making adjustments to care plans as needed to achieve optimal outcomes.
Requirements
  • Current RN License: Must be licensed to practice as a Registered Nurse in the state of practice.
  • CPR Certification: Must be certified in CPR.
  • Valid Driver's License: Must have a valid driver's license and current car insurance.
  • Minimum of One Year Experience: Must have a minimum of one year of relevant clinical RN experience.
  • Strong Computer and Technology Skills: Must have strong computer and technology skills.
