About the Role

We are seeking a skilled and compassionate Mental Health Support Worker to join our Home Treatment and Rapid Response team. As a key member of our team, you will work closely with patients and their families to provide evidence-based support and interventions in the comfort of their own homes.

Main Responsibilities

  • Conduct comprehensive assessments of clients referred to our Home Treatment and Rapid Response services
  • Develop and implement personalized support plans to promote recovery and resilience
  • Provide emotional support and guidance to clients and their carers
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Monitor client progress and adjust support plans as needed
  • Communicate effectively with clients, carers, and healthcare professionals to ensure optimal care
