We are seeking a skilled and compassionate Mental Health Support Worker to join our Home Treatment and Rapid Response team. As a key member of our team, you will work closely with patients and their families to provide evidence-based support and interventions in the comfort of their own homes.
Main Responsibilities
- Conduct comprehensive assessments of clients referred to our Home Treatment and Rapid Response services
- Develop and implement personalized support plans to promote recovery and resilience
- Provide emotional support and guidance to clients and their carers
- Collaborate with multidisciplinary teams to ensure seamless care coordination
- Monitor client progress and adjust support plans as needed
- Communicate effectively with clients, carers, and healthcare professionals to ensure optimal care
