About the Role

We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team at NELFT North East London Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care and support to children and young people with mental health challenges, learning disabilities, and autism spectrum disorder.

Key Responsibilities
  • Conduct detailed mental health assessments for young people, including the identification and management of risk factors.
  • Work as part of a multi-disciplinary team to provide comprehensive care and support to children and young people and their families.
  • Maintain a caseload of young people, working in an evidence-based and time-limited way, including the use of outcome monitoring to measure progress and client satisfaction.
  • Develop and implement individualized care plans that address the unique needs and goals of each young person.
  • Collaborate with other healthcare professionals, including psychologists, social workers, and psychiatrists, to ensure comprehensive care and support.
  • Provide education and training to students and other healthcare professionals on mental health issues and best practices.
Requirements
  • RMN/LD Post Registration Qualification to Degree level or equivalent.
  • Relevant experience of working therapeutically with children/young people and their families.
  • Experience of assessing and managing risk factors.
  • Mentorship Qualification (or relevant experience of working with student nurses and willingness to attend training).
  • Experience of holding clinical responsibility for managing a caseload.
