Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to join our team at Farnham Park Health Group. The successful candidate will be an enthusiastic and committed professional with a strong background in primary care.
Main Responsibilities- Support General Practitioners in delivering high-quality patient care using advanced autonomous clinical skills.
- Assist with same-day triage, assessment, diagnosis, and treatment of patients presenting with a wide range of acute illnesses.
- Manage and care for patients with long-term conditions, prescribing and reviewing medication as necessary.
- Provide information and advice on prescribed medications, side effects, and interactions.
- Maintain a flexible approach and work collaboratively with other clinical team members.
- Registered General Nurse with the NMC.
- Minimum of 5 years post-registration experience.
- Recent experience in primary care, community, or emergency settings.
- Ability to assess and manage patient risk effectively and safely.
- Excellent interpersonal, verbal, and written communication skills.
- Recognized ANP/MSC qualification at a minimum of degree level or equivalent.
- Independent prescriber.
- Development of areas of clinical expertise.
