Care Team Manager

We are seeking an experienced Team Manager to join our Adult Countywide Living in Care Home Review Team. The successful candidate will be responsible for leading a team of professionals who support adults with physical disabilities and older people, alongside their families and informal carers.
Key Responsibilities- Lead a team of professionals in providing high-quality care and support services to adults with physical disabilities and older people.
- Develop and implement effective care plans that meet the needs of service users.
- Work closely with care homes and other stakeholders to ensure seamless delivery of care services.
- Manage and develop the skills and knowledge of team members to ensure they are equipped to provide excellent care.
- Collaborate with other teams and services to ensure a comprehensive approach to care and support.
