Primary Care Mental Health Specialist

Our Primary Care Mental Health Service is a pioneering initiative that has garnered national recognition for its innovative approach to mental health care in Primary Care settings. By embedding our team within GP practices and Neighbourhood teams, we ensure that individuals with mental health needs receive timely and appropriate support. We are seeking a highly motivated and experienced Band 6 Primary Care Mental Health Practitioner to join our team, who will work closely with Mental Health Liaison Practitioners to facilitate mental health assessments in Primary Care and support GPs in joint decision-making regarding secondary care referrals, signposting, and transition of service users from secondary mental health services back to Primary Care.
As a key member of the primary care team, you will provide support, education, and problem-solving approaches for adults of working age, helping to inform GPs' management plans for those with mental health needs in Primary Care. This is a dynamic and autonomous role that will allow you to develop your expertise, decision-making skills, and service user focus.
Main Responsibilities:
- Working alongside Mental Health Liaison Practitioners to facilitate mental health assessments in Primary Care and support GPs in joint decision-making regarding secondary care referrals, signposting, and transition of service users from secondary mental health services back to Primary Care.
- Providing support, education, and problem-solving approaches for adults of working age, helping to inform GPs' management plans for those with mental health needs in Primary Care.
- Developing close relationships with Primary Care practices, attending practice meetings, and having regular informal contact.
- Delivering interventions under Primary Care governance and policies while maintaining adherence to CPFT Trust policy.

Key Requirements:
- Registered mental health professional, appropriate to the job role (nursing, OT, SW).
- Completion of Mentorship/preceptorship Course/ENB equivalent, or intention to complete.
- Evidence of continuous professional development.
Desirable Requirements:
- Non-medical prescriber.
Experience:
- Experience of working alongside people with complex health problems in the designated care group.
- Experience of supervising staff.
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems.
- Experience of working consultatively with professionals.
- Experience of conducting clinical assessments, including risk assessments.
- Experience of partnership working within and across various statutory and non-statutory teams and agencies.
Skills & Abilities:
- Excellent negotiation and conflict resolution skills.
- Ability to work with resistance at times, to manage own reactions to difficult situations and to act with appropriate authority and diplomacy.
- IT literacy and willingness to take on new technology.
Knowledge & Understanding:
- Significant post-qualifying experience in the relevant care group.
Physical Requirements:
- Ability to use manual handling techniques.
- Ability to travel across sites.
