We are seeking an experienced and compassionate Social Worker to join our team. As a Social Worker, you will work closely with adults with learning disabilities, their families, and carers to assess needs, develop care plans, and ensure they receive the necessary support to enhance their quality of life.
Key Responsibilities:
- Conduct comprehensive assessments to identify the needs and strengths of adults with learning disabilities.
- Develop and implement personalized care plans that promote independence, well-being, and social inclusion.
- Collaborate with multi-disciplinary teams, external agencies, and community organizations to coordinate care and services.
- Ensure all interventions comply with relevant legislation, policies, and procedures related to adult social care.
- Build strong, trusting relationships with individuals, their families, and carers to support their long-term needs.
Requirements:
- Qualified Social Worker registered with Social Work England.
- Experience working with adults with learning disabilities.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
