Job Summary

We are seeking a highly motivated and experienced Community Health Nurse to join our team at South Warwickshire University NHS Foundation Trust. As a Community Health Nurse, you will play a key role in delivering high-quality healthcare services to children and families in the community.

Key Responsibilities
  • Provide comprehensive health assessments and interventions to children and families in the community.
  • Develop and implement individualized care plans to meet the unique needs of each family.
  • Work closely with other healthcare professionals to ensure seamless care delivery and coordination.
  • Participate in health promotion programs and services to improve health outcomes for children and families.
  • Collaborate with other agencies and organizations to ensure effective service delivery and coordination.
Requirements
  • Registered Nurse (RGN/RSCN Adult/Child) qualification.
  • Post-registration experience of at least 12 months.
  • Ability to work collaboratively with children, young people, families, and other professionals.
  • Strong communication and interpersonal skills.
  • Ability to prioritize work and manage multiple tasks and responsibilities.
  • Basic IT skills and ability to use Microsoft Office/Word.
  • Knowledge of public health role within Health Visiting and issues affecting children and young people.
  • Ability to maintain confidentiality and handle sensitive information.

Working Arrangements
  • Full-time position.
  • 37.5 hours per week.
  • Monday to Friday, 8am-5:30pm.
  • Must have a full driving license and access to a vehicle to undertake community role.
