About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Home Treatment Team in Chester. The successful candidate will be responsible for providing comprehensive assessments and interventions to individuals presenting in crisis, working as part of a multi-disciplinary team to deliver high-quality care.

Main Responsibilities
  • Conduct detailed assessments of individuals' needs and provide evidence-based interventions to promote recovery and well-being.
  • Work collaboratively with other healthcare professionals to develop and implement individualized care plans.
