About the Role
We are seeking a highly skilled and experienced professional to join our Urgent Community Response team as a Community Health Specialist. This is an exciting opportunity to work in a dynamic and fast-paced environment, providing expert clinical interventions and rehabilitation to patients in their own homes.
Key Responsibilities
- Provide comprehensive specialist assessments, reviews, and evaluations of patient needs and develop personalized care plans.
- Lead urgent community responses to patients in crisis, preventing hospitalizations and ensuring safe care at home.
- Collaborate with multidisciplinary teams to allocate workloads and ensure timely delivery of care.
- Develop and maintain partnerships with healthcare professionals, community organizations, and social services to enhance patient outcomes.
- Contribute to the development of clinical pathways and best practices in community care.
Requirements
- Post-registration experience in relevant clinical areas, such as care of the elderly, orthopedics, neurology, or palliative care.
- Proven experience working in multidisciplinary teams and collaborating with healthcare professionals.
- Excellent communication, leadership, and problem-solving skills.
- Ability to work independently and as part of a team.
- Valid and current driving license and ability to travel throughout Buckinghamshire.
