About the Role

We are seeking a skilled and compassionate Community Healthcare Assistant to join our Rapid Response & Intermediate Care Team. As a key member of our team, you will provide high-quality care and support to adults in their own homes, promoting independence and enabling them to remain safely in their own environment for as long as possible.

Key Responsibilities
  • Provide general and specific care to adults within competencies
  • Work as an integral member of the Rapid Response & Intermediate Care (RRIC) team, delivering defined care including nursing, social, and therapy-based rehabilitative care to patients in liaison with senior staff
  • Take responsibility for designated elements of patient treatment protocols and delivery of such treatments in patients' own homes
  • Complete and potentially delegate non-clinical tasks within the team
  • Improve the care and support of patients in the community, enabling them to remain safely in their own home for as long as possible
  • Develop an innovative approach, excellent communication skills, and a willingness to work with other members of the community teams to ensure high-quality service delivery
  • Be willing to further develop your clinical expertise in a range of areas
