Position Overview:
As a Community Rehabilitation Specialist, you will be an integral part of a multidisciplinary team dedicated to providing comprehensive health and social care services to individuals in need.

Role Details:
Job Title:
Band 6 Rehabilitation Specialists

Key Responsibilities:
  • Deliver expert assessments and therapeutic interventions primarily in community settings, with occasional responsibilities in inpatient environments.
  • Engage in effective communication with patients, families, and caregivers, ensuring the delivery of complex and sensitive information.
  • Collaborate across various organizational boundaries, working alongside statutory, third, and voluntary sector entities to meet the health and social care needs of individuals.
  • Manage referrals, request medical support, and participate in joint service development initiatives.
  • Occasionally assume a rapid response role to conduct assessments aimed at preventing hospital admissions and facilitating timely discharges.
