About the Role:

We're looking for a Support Worker who can provide high-quality care and support to our service users. As a key member of our team, you'll work closely with our service users to develop and implement personalized support plans, promoting their independence, confidence, and well-being.

Key Responsibilities:

  • Provide person-centred support to service users, promoting their independence and well-being
  • Develop and implement personalized support plans, working closely with service users and their families
  • Support service users with daily living tasks, such as personal care, meal preparation, and household chores
  • Encourage and support service users to engage in activities that promote their physical and mental health
  • Work collaboratively with our multidisciplinary team to ensure seamless care and support

Requirements:
