CWP is seeking an experienced Registered General Nurse to join our Care Community Teams as a Community Health Nurse. As a key member of our team, you will play a vital role in delivering high-quality nursing services to patients in their own homes.
Key Responsibilities:
- Deliver holistic assessments of patients' health needs using a person-centred approach.
- Work alongside various professionals to provide seamless care to patients.
- Coordinate patients' care needs, avoiding unnecessary admission to hospital or long-term care settings.
- Support our teams to deliver person-centred care.
