We are seeking an experienced mental health professional to join our team as an ARMS Care Coordinator in our Early Intervention in Psychosis Service. As a key member of our multi-disciplinary team, you will be responsible for providing care coordination for individuals with At Risk Mental State and First Episode Psychosis.
Key Responsibilities- Assess, plan, implement, and evaluate the care needs of patients under the care of the Early Intervention Service.
- Act as care coordinator for patients identified as having At Risk Mental State and First Episode Psychosis.
- Work collaboratively with the multi-disciplinary team to facilitate recovery and promote well-being.
- Develop and deliver evidence-based interventions to optimize recovery and prevent relapse.
The Black Country EIP service is comprised of four EIP teams located in Dudley, Walsall, Sandwell, and Wolverhampton. We are a specialist community mental health team working with young people and adults in the three years following First Episode Psychosis, or those who are deemed at risk of developing psychosis for a year.
We provide individualized, comprehensive, evidence-based interventions to optimize recovery, prevent relapse, and help individuals and their families to cope with their experiences. In doing so, we aim to improve social, physical, psychological, and vocational functioning and reduce the short and long-term impact of psychosis in line with the EIP Care Pathway.
Requirements- Registered professional RMN, Social worker, or OT.
- 18 months post-registration experience.
- Experience of working with psychosis or at risk mental state.
- Full UK driving license and access to a car for work.
