About the Role

We are seeking a highly skilled and compassionate Primary Care Nurse to join our team at South West Yorkshire Partnership NHS Foundation Trust. As a Primary Care Nurse, you will play a vital role in delivering high-quality patient care in community-based settings.

Key Responsibilities
  • Assess, plan, implement, and evaluate care for patients in their own homes and community-based settings.
  • Work in partnership with other health professionals to provide holistic patient care.
  • Develop and maintain effective relationships with patients, carers, and other healthcare professionals.
  • Participate in education and staff development, actively participating in regular supervision and appraisals.
  • Ensure compliance with clinical governance, including infection control policies and procedures.
  • Contribute to the development and implementation of policies, procedures, and guidelines relevant to your area of work.
Requirements
  • Registered Nurse with current registration with the NMC.
  • Experience in community-based care, preferably in a rehabilitation setting.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of government legislation and documentation affecting NHS and primary care.
Desirable
  • Experience in intermediate care.
  • Evidence of involvement in service or project development.
