Position Overview

The Universal Crisis Team is currently seeking Full-Time Community Support Specialists to join our dedicated team focused on the Older Adults pathway. We are in search of enthusiastic and committed professionals to enhance our service delivery.

The successful candidate will be integral to a team operating under a crisis intervention model, providing essential support within the WAA pathway. A proven history of clinical experience in managing complex cases is essential for this role.

This position presents an exciting opportunity for the right individual to become part of a skilled, passionate, and driven team.

Key Responsibilities

The Community Support Specialist will be responsible for:

  1. Conducting thorough assessments and managing casework for service users with significant and intricate needs.
  2. Offering specialized clinical guidance to service users, their families, staff, statutory bodies, third-sector partners, and the general public.
  3. Collaborating with service users and their families to create Recovery and Wellbeing-focused Care Plans, ensuring regular evaluations to track progress and adjust plans as needed.
  4. Recognizing the needs of caregivers and ensuring their involvement in the Care Planning process while delivering evidence-based clinical and psychological interventions.
  5. Working as part of a Multi-disciplinary Team to ensure the delivery of high-quality, individualized care that prioritizes safety, recovery, and independence.
  6. Coordinating treatment planning, interventions, reviews, and discharge processes.
  7. Providing compassionate care rooted in empathy, respect, and dignity.
  8. Incorporating a client-centered approach into all aspects of practice and collaborative care.

Qualifications

Essential:

  • First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent qualification in Social Work, Occupational Therapy, Clinical Psychology, or related fields.
  • Evidence of post-registration training in clinical or psychological interventions relevant to the role.
  • Demonstrated post-registration clinical experience.
  • 12 to 24 months of experience as a Band 5 qualified nurse.

Desirable:

  • Mentorship training.

Knowledge Requirements

Essential:

  • Understanding of professional accountability, NMC standards, and relevant local and national initiatives.

Desirable:

  • Familiarity with managing challenging behaviors.

Skills Required

Essential:

  • Ability to conduct comprehensive assessments of complex mental health needs.
  • Skills in risk assessment and management.
  • Capability to identify treatment options in accordance with NICE Guidelines.
  • High proficiency in written and verbal communication.
  • Ability to perform effectively under pressure.

Desirable:

  • Clinical leadership skills.
  • Delegation abilities.
