Job Title: Senior Community Nurse

We are seeking a highly skilled and experienced Senior Community Nurse to join our Community Response Team in Stratford. As a key member of our team, you will be responsible for providing high-quality care to patients in their homes, aiming to prevent unnecessary hospital admissions and support the facilitation of discharges from hospital.

The successful candidate will have a strong background in community nursing, with experience in assessing and managing the health and care needs of patients with complex conditions. You will work closely with the multidisciplinary team, including therapists, GPs, social services, and urgent response, to deliver seamless care across the locality.

Key responsibilities will include:

  • Assessing and managing the health and care needs of patients in their homes
  • Providing end-of-life support and delivering holistic, comprehensive care to patients in their own homes
  • Working closely with the multidisciplinary team to deliver seamless care across the locality
  • Deputising on behalf of the Band 7 Professional Lead and providing coordination for the team
  • Providing clinical leadership and guidance to junior staff and students

We are looking for a candidate who is a registered nurse with a degree-level qualification or equivalent knowledge. You will have a strong background in community nursing, with experience in assessing and managing the health and care needs of patients with complex conditions. You will also have excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
