Position Overview

We are pleased to announce a remarkable opportunity to join the Mental Health Intensive Support Service. This innovative program is dedicated to providing specialized evaluation, treatment, interventions, and support aimed at assisting individuals in overcoming intricate mental health challenges and rebuilding the skills and confidence necessary for successful community living.

As a Wellness Lifestyle Advisor, your role will focus on collaborating with patients within the community to advocate for healthy living practices that empower them to reach their personal health objectives.

Key Responsibilities

In your capacity as a Wellness Lifestyle Advisor, you will engage with patients in the community rehabilitation team to:

  • Evaluate their current lifestyle and overall well-being.
  • Establish health improvement goals.
  • Develop actionable plans and provide hands-on support and information to facilitate behavioral changes.

Your responsibilities will include promoting the advantages of physical activity, balanced nutrition, reduced alcohol consumption, safe sexual practices, smoking cessation, and more.

You will assist individuals in recognizing how their lifestyle choices may impact their health and guide them in formulating a personalized health strategy.

This position will also require you to take the initiative in organizing and managing the physical health clinic, which involves:

  • Monitoring vital health indicators (e.g., blood pressure, weight, height, BMI).
  • Conducting blood tests and ECGs.
  • Following up on health outcomes and coordinating with primary care and medical professionals as necessary.
