Job Title: Band 5 Children's Community Staff Nurse

We are seeking a highly skilled and dedicated Band 5 Children's Community Staff Nurse to join our team at Central and North West London NHS Foundation Trust. As a key member of our Community Children's Nursing Team, you will play a vital role in providing high-quality care to children and young people with complex needs.

Main Responsibilities:
  • Provide case management of an identified caseload of children and young people with complex needs, utilizing evidence-based care and promoting empowerment and partnership working.
  • Develop, implement, and evaluate care plans, ensuring the needs of children and young people are met in a safe and effective manner.
  • Work in partnership with parents, carers, and multi-disciplinary agencies to provide skilled care to children and young people within the community, supporting safe discharge and hospital admission avoidance.
  • Manage complex discussions with a range of agencies and individuals, including care providers, patients, and relatives, utilizing excellent interpersonal skills.
  • Contribute to the culture of the team, promoting inclusivity, kindness, and a hardworking environment.
Requirements:
  • Registered Children's Nurse RSCN qualification.
  • Numeracy skills required for the safe administration of medicines.
  • Experience as a Band 6 or Band 5 nurse, with recent experience within an acute or community setting.
  • Ability to communicate effectively with the multi-disciplinary team, present cases, and use handover effectively.
  • Understanding of the Mental Capacity Act and Deprivation of Liberty Safeguards, and the Mental Health Act.
  • Ability to work autonomously, prioritize workload, and manage complex situations.
