Job Summary:We are looking for a permanent part-time Practice Nurse who is committed to delivering high-quality patient care and is passionate about working in a dynamic and supportive team environment.
Main Responsibilities:- Provide and maintain a high standard of nursing care for patients in line with current evidence-based guidelines.
- Treat and review patients, referring as necessary, and work within a multi-disciplinary team to prevent unplanned hospital admissions.
- Manage long-term conditions and participate in health promotion and disease prevention activities.
- Assist GPs with minor surgery and coil fittings, and provide support with routine immunisations and child immunisations.
- Participate in administrative systems and maintain accurate and contemporaneous records.
- Clinical: Provide wound care, ulcer care, Doppler, removal of sutures/clips, urinalysis, ECGs, chronic disease management, and minor illness care.
- Special Requirements: Maintain confidentiality, adhere to the Qualities and Outcomes framework, and use own judgment and resourcefulness.
- Confidentiality: Respect patient confidentiality and maintain confidentiality in all aspects of work.
- Personal/Professional Development: Participate in training programs, maintain a record of personal and professional development, and contribute to the effectiveness of the team.
- General: Promote and maintain health, safety, and security, identify risks, and report potential risks.
- Registered Nurse (Nursing and Midwifery Council)
- Independent prescriber
- Post-Graduate Diploma or degree in Nursing (Nurse Practitioner)
- Desirable: Experience in Primary care, Chronic disease Management, and working as a Practice Nurse or Community Nurse
