Job Title: Community Health Nurse
Job Summary:
We are seeking a highly skilled and experienced Community Health Nurse to join our team at Berkshire Healthcare NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our team.
Key Responsibilities:
- Case Management: Operate within a Case Management framework to coordinate care for an identified caseload of patients.
- Patient Assessment and Care Planning: Assess patient need and plan, implement, and evaluate programs of nursing care.
- Preventative Measures: Identify and plan preventative measures and anticipatory care needs.
- End of Life Care: Manage End of Life Care and work in partnership with health, social, and voluntary agencies to ensure patients are treated with dignity and respect.
- Leadership and Team Management: Demonstrate leadership skills and manage a team of nurses to ensure seamless service delivery.
- Training and Development: Ensure Mandatory and Statutory training is booked and completed within time scales, and manage team training and development.
- Annual Leave and Service Continuity: Manage annual leave and ensure continuity of service by ensuring team members take regular annual leave.
Requirements:
- Education and Qualifications: RGN, Teaching qualification, or willingness to undertake training, and evidence of ongoing professional development.
- Experience: Minimum 2 years recent community nursing experience, Primary Care Experience, and Case Management skills.
- Skills and Knowledge: Ability to effectively manage challenging behavior and conflict, demonstrate understanding of the Aging Well agenda, and computer literacy.
