We are seeking an experienced Advanced Clinical Research Practitioner to join our team at Tonbridge Primary Care Network. As an Advanced Clinical Research Practitioner, you will play a key role in delivering high-quality patient care and contributing to the development of our primary care services.
About the RoleThe successful candidate will be responsible for working in our Same Day Access Hub, seeing patients for urgent on-the-day issues. You will work alongside our PCN team of GP supervisors and paramedic practitioners to deliver safe and high-quality care. You will also be involved in cross-cover with our award-winning Frailty Visiting Team, visiting patients who are housebound for urgent on-the-day issues.
Key Responsibilities- Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with acute, long-term, and complex health needs.
- Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology, taking appropriate action.
- Triage and treat patients, including same-day triage, performing assessments of their needs and making referrals as necessary to other members of the primary and secondary healthcare teams.
- Interpret investigation findings to determine effective treatment plans and discuss with GPs where a second opinion is required.
- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition, using both non-pharmacological and pharmacological treatments.
- Co-ordinate the planning and delivery of care, ensuring patients and carers or relatives are involved where required.
- A clear understanding of the Multi-professional framework for advanced clinical practice in England.

- Masters degree level in the relevant area of expertise.
- For Advanced Clinical Practitioner Nurses only, have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route.
- MSc in advanced practice or equivalent accreditation.
- For advanced practice pharmacists only, qualified from an approved 18-month training pathway or equivalent and qualified as an independent prescriber.
- Clinical supervision training and experience.
