We are seeking a highly skilled and dedicated Social Care Specialist to join our team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our Community Social Care Resource, you will play a vital role in providing assessment, support planning, and care duties under the Care Act.
Key Responsibilities- Ensure evidence-based practice and research awareness is used as the foundation for assessment, planning, implementation, and evaluation of care.
- Engage with service users and their families/carers, developing therapeutic relationships, and providing evidence-based, psychosocial interventions in various settings.
- Act as care coordinator for an agreed number of service users, adopting a team approach to provide interventions to other service users, families/carers as appropriate.
- Assess complex health and wellbeing needs, develop, monitor, and review care plans to meet specific needs.
- Undertake initial and ongoing risk assessments, utilizing standardized tools, to develop accurate and appropriate risk management plans and interventions, highlighting service user strengths, and positive risk options.
- Develop and coordinate integrated team working with teams, and other agencies involved in the care of the individual, their families/carers.
- Promote service user and carer involvement; ensuring choices are embedded into the philosophy of care.
- Empower people to address specific needs in relation to health and wellbeing, and enable individuals to become expert in managing their condition.
- Ensure social work perspectives and values are supported and represented within the multidisciplinary approach.
