Independent travel is a necessary requirement for this position.

Key Responsibilities

Understand and embody the Trust's vision, translating it into actionable strategies for daily operations within a community context. Support Caseload holders and Team Leaders in promoting the Trust's vision, fostering a culture of modern, customer-focused services that prioritize effective patient-centered care. Provide nursing care to patients in their homes or preferred residences, offering alternatives to hospital admissions, facilitating timely discharges, and empowering individuals to achieve their potential.

We are committed to providing clinical support, continuing professional development, and training opportunities.

The role operates within a 7-day service from 8 am to 6 pm, following a Rota System.
