We are seeking a skilled Health Visitor to join our team at Hunter AHP. As a Health Visitor, you will play a vital role in promoting health and wellbeing within the community. You will be responsible for managing caseloads and delivering the Healthy Child programme, ensuring exceptional care for families and individuals.
Key Responsibilities:
- Conduct maternal mood assessments and new birth visits to identify and address potential health issues
- Champion child safeguarding and promote overall wellbeing through education and support
- Collaborate effectively with multi-agency teams to ensure comprehensive care for families and individuals
Requirements:
- Postgraduate Diploma or Certificate in SCPHN Health Visitor
- Current NMC Registration
- Proficient in computerized recordkeeping and Excel
- Strong communication skills and ability to work in diverse teams
- Commitment to continuous professional development
