In this pivotal role, you will be instrumental in guiding a team of both Trainee and qualified Advanced Clinical Practitioners (ACPs). Your main objective will be to create a cohesive frailty care pathway that transcends organizational boundaries, ensuring exceptional care for our elderly patients.

Key Responsibilities
The successful candidate will be expected to operate independently to:
