Job Summary

This role is an exciting opportunity to join our team as a Senior Rehabilitation Support Worker, working in a dynamic and patient-centered environment. You will be part of a multidisciplinary team, providing high-quality care and support to adults with physical disabilities, promoting independence and maximizing functional abilities.

About the Role

As a Senior Rehabilitation Support Worker, you will be responsible for assessing, planning, implementing, and reviewing individual rehabilitation and care programs for clients. You will work closely with occupational therapists, physiotherapists, nurses, and social workers to ensure comprehensive care and support.

Key Responsibilities
  • Contribute to the assessment, planning, implementation, and review of individual rehabilitation and care programs for clients.
  • Independently implement rehabilitation and care programs as appropriate in the patient's property.
  • Teach prescribed exercise programs, practice transfers and mobility, assist and promote independence in personal care and domestic tasks.
  • Undertake access visits to feedback environmental information to therapists and document interventions accurately in notes following Trust guidelines.
  • Monitor clients' progress and amend programs independently to maximize rehabilitation progression and patient safety.
Person Specification

We are looking for an individual with relevant experience working in a health, education, or social care setting, with a strong understanding of physical and psychological conditions and their impact on daily activities. You should be competent in the use of basic IT, including MS Word, email, and internet, and have excellent communication and negotiation skills.

A UK/EEU driving license is desirable, and experience of working in a rehabilitation setting, community/lone working, and contributing to service development or training is also advantageous.
