Job Summary:

Active Pathways is seeking a dedicated and compassionate Registered Mental Health Nurse to join our team at Brookhaven, a 23-bedded open-rehabilitation service for adults with enduring mental health needs.

About the Role:

We are looking for a skilled and empathetic professional to work within our Recovery and Social Inclusion Programme, which focuses on inclusion, skills development, and consolidation, risk reduction, and relapse prevention.

Key Responsibilities:

  • Deliver high-quality care and support to service users, promoting their recovery and independence.
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each service user.
  • Conduct risk assessments and complete care plans, ensuring a holistic approach to promoting recovery and independence.
  • Work collaboratively with the multidisciplinary team to ensure seamless care and support.
  • Participate in ongoing learning and development opportunities to enhance skills and knowledge.

Requirements:

  • Active PIN and Nursing qualification.
  • Experience within a Mental Health or Rehabilitation setting.
  • Understanding of the importance of delivering a holistic approach to promoting recovery and independence.
  • Ability to write care plans and complete risk assessments.
  • Good understanding and ability to adhere to health and safety guidelines.
