About the Role

We are seeking a dedicated and compassionate Care Team Leader to join our team at our beautiful new care home in Lisvane, Cardiff. As a Care Team Leader, you will play a vital role in ensuring the highest standards of care and support for our residents.

Key Responsibilities
  • Lead and supervise your shift, ensuring that all residents receive the best possible care and support.
  • Case manage residents and liaise with families, healthcare professionals, and other stakeholders to create and deliver personalized care plans.
  • Administer and record medication, ensuring accuracy and attention to detail.
  • Carry out risk assessments and work closely with the leadership team to develop services that meet the needs of our residents.
  • Work alongside care assistants to support residents with daily needs, promoting independence and well-being.

About You

We are looking for someone who is well-organized, IT literate, and able to allocate and delegate work effectively. You should have a proven ability to lead and supervise others, a good understanding of care planning processes, and experience in administering medication. Above all, you should have a natural ability to help make the lives of others more fulfilling.
