Job Title: Children and Young People's Family Worker

Job Summary:

We are seeking a dedicated and compassionate Family Worker to join our team at Connect2Halton. As a key member of our team, you will play a vital role in supporting families with children and young people who require early help and intervention.

Key Responsibilities:

  • Establish and maintain relationships with families, understanding their needs and concerns.
  • Work collaboratively with other professionals to provide a holistic and person-centered approach to supporting children and families.
  • Conduct MAP and Short Breaks assessments, coordinate services, and implement and review action plans.
  • Ensure the voice of the child influences their support plan.
  • Travel to visit families in their own homes and elsewhere in the community.

Requirements:

  • Working knowledge of relevant government legislation, guidance, and local procedures relating to disabled children, their carers, and safeguarding.
  • Experience of working collaboratively with colleagues from other organizations, including social care, health, education, commissioned services, and the voluntary sector.
  • Ability to travel, sometimes at short notice, and transport children and young people to interventions as required.
