Halton's Short Break Service for Children with Disabilities is seeking a dedicated and enthusiastic professional to join our team. As an Early Help Family Worker, you will play a key role in supporting families of children with disabilities to access services that meet their needs.
You will have a working knowledge of relevant government legislation, guidance, and local procedures relating to disabled children, their carers, and safeguarding. Your experience of working collaboratively with colleagues from other organisations, including social care, health, education, commissioned services, and the voluntary sector, will be invaluable in this role.
The Early Help Family Worker role includes leading professionals to complete MAP and Short Breaks assessments, coordinating services, and implementing and reviewing action plans. You will ensure that the voice of the child influences their support plan and work with other professionals to provide a holistic and person-centred approach to supporting the child and their family.
You will manage an individual caseload and be responsible for ensuring that the needs of children with disabilities are met. Your ability to work effectively with families, professionals, and other stakeholders will be essential in achieving this goal.
Key Responsibilities:
- Lead professionals to complete MAP and Short Breaks assessments
- Coordinate services to meet the needs of children with disabilities
- Implement and review action plans to ensure the voice of the child is heard
- Work collaboratively with other professionals to provide a holistic and person-centred approach to supporting the child and their family

Requirements:
