Job Title: Diabetes Nurse

We are seeking a highly skilled and experienced Diabetes Nurse to join our team at Crossgates Medical Practice. As a Diabetes Nurse, you will play a vital role in managing and supporting patients with diabetes, working independently and as part of a team to deliver high-quality patient care.

About the Role:
  • Provide specialist nursing care for patients with diabetes, including education, monitoring, and treatment.
  • Work closely with the GPs, ANP, Practice Nurses, and other team members to provide integrated care.
  • Carry out regular monitoring of patients' diabetes condition, including making changes to their medication and maintaining accurate patient records.
  • Support patients with prevention strategies, lifestyle changes, and initiate treatment for newly diagnosed patients.
About Us:

We are a small, close-knit GP surgery based in Crossgates, Fife, with a practice population of approximately 4,500 patients. Our practice team is comprised of 2 GP Partners, an ANP, a Practice Nurse, Practice Manager, and 7 admin staff. We also have a number of attached staff supporting us as part of the wider Multi-Disciplinary Team.

Key Responsibilities:
  • Provide comprehensive diabetes management, including education, monitoring, and treatment.
  • Work collaboratively with the healthcare team to deliver high-quality patient care.
  • Maintain accurate patient records and make changes to medication as necessary.
  • Support patients with prevention strategies and lifestyle changes.
