Adult Nurse Manager Opportunity

As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans that meet their unique needs.

Key Responsibilities:
  • Design and deliver tailored care plans that address the physical and physiological needs of clients
  • Effectively manage the multidisciplinary team through strong communication and teamwork
  • Provide ongoing monitoring, support, and assessment of clients' needs
  • Deliver nursing interventions, including medication management
  • Build and maintain relationships with clients and their families
  • Collaborate with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages
Requirements:
  • RGN with an active NMC PIN
  • Supportive management style that fosters a positive team environment
  • Up-to-date experience of tracheostomy care and ventilators within the last 2 years
  • Advanced communication skills for effective liaison with patients, families, and the MDT
  • Suitable for Band 5/6 Nurses with evidence of effective leadership and management of HCAs
  • Full UK driving license desirable but not essential
