Randstad Care is seeking a dedicated Social Worker to join our Hospital Discharge Team in Newcastle. As a key member of our team, you will play a vital role in supporting patients transitioning from hospital to home.

Key Responsibilities:

  • Assess patient needs: Conduct thorough assessments to identify patients' needs and develop personalized care plans.
  • Coordinate discharges: Work closely with healthcare professionals to ensure safe and timely discharges from the hospital.
  • Provide support: Offer emotional support and guidance to patients and their families during the transition process.
  • Collaborate with teams: Liaise with healthcare teams, social services, and other stakeholders to ensure seamless care coordination.

Requirements:

  • Experience: Proven experience in hospital discharge planning or a related field.
  • Qualifications: Relevant qualifications in social work or a related field.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
