Job Title: Support Coordinator

We are seeking a highly skilled and dedicated Support Coordinator to join our team at Creative Support. As a Support Coordinator, you will play a vital role in providing outcome-focused care and support to individuals who are at risk of homelessness and in need of housing-related provision.

About the Role

This is an exciting opportunity to work with a passionate and inclusive organization that is committed to promoting inclusion, independence, and wellbeing. As a Support Coordinator, you will be responsible for managing a team of recovery workers and providing direct support to service users in accordance with their holistic support plans.

Key Responsibilities
  • Manage a team of recovery workers to provide direct support to service users
  • Develop and implement individualized support plans to meet the needs of service users
  • Provide one-off interventions and longer-term support to service users
  • Work collaboratively with other teams to ensure a seamless service delivery
  • Contribute to the development of policies and procedures to ensure the highest standards of care and support
Requirements

To be successful in this role, you will need:

  • Experience of working in a care and support setting
  • Excellent communication and interpersonal skills
  • Ability to work flexibly, including evenings and weekends
  • Access to a car
  • Knowledge of local service provision and community resources
