Support Coordinator

We are seeking a highly skilled and experienced Support Coordinator to join our team at Creative Support. As a Support Coordinator, you will play a vital role in providing outcome-focused care and support to individuals who are at risk of homelessness and in need of housing-related provision.
Key Responsibilities- Manage a team of recovery workers and provide direct support to service users in accordance with their holistic support plans.
- Provide one-off interventions as well as longer-term support, devising clear exit strategies that will help service users build resilience, local connections, and provide them with knowledge of local service provision.
- Work collaboratively with the team to ensure that service users receive a high-quality, person-centered service.
- Contribute to the development and implementation of service plans, ensuring that they are tailored to meet the individual needs of service users.
- Monitor and evaluate the effectiveness of support plans, making adjustments as necessary to ensure that service users achieve their goals.
- Experience of working in a support or care role, preferably in a housing-related setting.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, colleagues, and external partners.
- Ability to work flexibly, including evenings and weekends, to meet the needs of the service.
- Knowledge of housing-related issues and the ability to provide support and guidance to service users.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality services.
