About the Role

We are seeking a highly skilled and dedicated Care Coordinator to join our team at the Central and North West London NHS Foundation Trust. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals experiencing a First Episode of Psychosis.

Key Responsibilities
  • Undertake comprehensive assessments of health and social care needs, working collaboratively with the multi-disciplinary team to develop and implement effective care plans.
  • Provide care coordination for a caseload of service users, ensuring their needs are met and they receive timely and appropriate interventions.
  • Work closely with primary care services to ensure seamless transitions and referrals.
  • Develop and maintain relationships with community and voluntary agencies to provide patients with alternative avenues for support and therapeutic help.
  • Ensure safe discharge processes and facilitate joint training events within the organization.
Person Specification
  • Registered Nurse RMN/Occupational Therapist/Social Worker with registration with the relevant professional body.
  • Significant experience of working in a clinical area providing mental and physical health care and assessment.
  • Experience of working with people in the community and understanding of care planning, safeguarding, and advocacy.
