Job Summary

We are seeking a motivated and enthusiastic clinical pharmacist to join our team in the community intermediate care unit. As a CHS Specialist Pharmacist, you will provide a high-quality clinical pharmacy and medicines optimisation service to patients, working closely with the multidisciplinary treatment team. This is an exciting opportunity to develop your skills and experience in a stimulating environment.

Main Duties

The successful candidate will be responsible for delivering a comprehensive medicines optimisation service, developing clinical knowledge and skills, and providing input into policy and procedures affecting the use of medicines. You will also participate in the pharmacy on-call service and contribute to the training and education of trainee pharmacists and junior pharmacists.

Person Specification

We are looking for a pharmacist with a strong interest in care of the elderly and intermediate care, who has experience in hospital pharmacy and is registered with the General Pharmaceutical Council. You should have good written and verbal communication skills, be able to work independently and as part of a team, and have a strong commitment to delivering excellent patient care.

Key Responsibilities

  • Deliver a comprehensive medicines optimisation service to patients and the multidisciplinary treatment team
  • Develop clinical knowledge and skills to practice competently as a foundation pharmacist
  • Provide input into policy and procedures affecting the use of medicines
  • Participate in the pharmacy on-call service
  • Contribute to the training and education of trainee pharmacists and junior pharmacists
