Nurse Specialist

We are seeking a highly motivated and patient-focused Long Term Care Specialist Nurse to join our team. As a key member of our NHS Long Term Care Service, you will be responsible for providing high-quality care to patients in care homes across the Hywel Dda University Health Board area.
Key Responsibilities- Assess and care for patients with ongoing care needs
- Develop and implement care plans in collaboration with multi-disciplinary teams
- Provide support and guidance to care home staff
- Work closely with secondary care teams to ensure smooth transitions and patient-focused care
- Contribute to the development of new care pathways and initiatives
- First Level Registered Nurse qualification
- Current NMC registration
- Evidence of continued personal and professional development to postgraduate diploma level or equivalent experience
- Commitment to lifelong learning
- Qualification in the care of Older Adults
- Welsh Speaker (Level 1)
This is a full-time role based in either Carmarthen, Llanelli, or Pembroke Dock, with the possibility of flexible working arrangements. You will be required to travel across the Hywel Dda University Health Board area and neighbouring counties.
