Job Title: Dual Diagnosis Recovery Worker

About the Role

As a Dual Diagnosis Recovery Worker, you will play a vital role in providing support and guidance to individuals with dual diagnosis, ensuring they receive the care and treatment they need to achieve recovery. You will work closely with our team to develop and implement personalized care plans, providing a safe and supportive environment for our service users.

Main Responsibilities
  • Provide dedicated mental health and substance use support to individuals with dual diagnosis
  • Develop and implement personalized care plans to meet the unique needs of each service user
  • Work collaboratively with our team to ensure seamless care coordination and support
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed
  • Provide education and training to staff on dual diagnosis and related topics
  • Participate in regular case meetings and contribute to the development of service user plans
