About the Role

We are seeking an experienced and compassionate Staff Nurse to join our Hospice at Home Team, providing high-quality care to patients requiring end-of-life care in their own homes.

Key Responsibilities
  • Deliver individualized care to patients, including symptom control and medication management.
  • Work collaboratively with the multidisciplinary team to ensure a coordinated and defined management plan for each patient.
  • Communicate effectively with patients, families, and healthcare professionals to ensure a safe and needs-led care pathway.
  • Maintain accurate healthcare records and participate in training and development opportunities.
Requirements
  • Registered Nurse with experience in palliative care and end-of-life care.
  • Knowledge and understanding of current developments in palliative care.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and adhere to data protection policies.
