We are seeking a highly skilled and organized individual to join our Somerset Hub for Coordinating Care team as a Patient Transfer Facilitator. This exciting opportunity will involve working closely with clinicians to facilitate a smooth referral process for service users and patients, providing a single point of contact for information on patients to be transferred / admitted to community hospital beds within the Primary Care Trust.
Key Responsibilities- Support the Somerset Hub for Coordinating Care clinicians in providing an efficient referral process, to include booking transport and liaising with patients.
- Work across multiple computer systems, coordinate telephone calls, and complete a range of reports.
- Develop strong relationships with clinicians and other stakeholders to ensure seamless communication and coordination.
- Excellent written and verbal communication skills.
- Experience in the use of IT systems.
- ECDL / ability to use Microsoft Word, Excel, and Outlook.
