About the Role

We are seeking an experienced Advanced Practice Physiotherapist to join our team at Connect Health Group Limited. As a key member of our rheumatology service, you will be responsible for managing a complex clinical caseload and working closely with experienced consultants to deliver high-quality patient care.

Key Responsibilities

  • Manage a varied and complex clinical caseload, including non-inflammatory and persistent joint problems
  • Work directly with consultants to deliver one-stop clinics and streamline patient care
  • Implement individualized treatment plans, including pharmacological and non-pharmacological interventions
  • Triage referrals from GPs and direct patients effectively
  • Lead and participate in team meetings for holistic patient care
  • Provide specialty advice across the pathway
  • Engage in clinical research and advance medical knowledge
  • Deliver educational sessions to raise awareness of rheumatological conditions
