Care Coordinator

We are seeking an experienced Care Coordinator to join our dedicated team at Apollo Home Healthcare. As a key member of our team, you will play a vital role in the delivery of high-quality care to our service users.
Key Responsibilities- Manage a caseload of care packages and coordinate care delivery to meet the needs of our service users.
- Develop and maintain effective relationships with service users, their families, and healthcare professionals.
- Coordinate rotas for care teams and respond to day-to-day changes in a timely and efficient manner.
- Act as the first point of contact for service users, their families, and other healthcare professionals.
- Work collaboratively with the care team to ensure seamless care delivery.
- Experience in care coordination and management.
- Excellent communication and relationship-building skills.
- Ability to work well under pressure and adapt to changing situations.
- Valid driving license and access to a vehicle.
- Commitment to supporting the out-of-hours on-call service.
- Competitive salary between £29,460 - £35,460 per annum.
- Payment for on-call duties.
- Bonus scheme up to £2,000 per annum.
- 25 days annual leave, plus bank holidays.
