We are seeking a compassionate and dedicated Support Worker to join our Health and Care service. As a Support Worker, you will provide practical and emotional support to individuals in need, enabling them to live independently and maintain their dignity.
Key Responsibilities- Provide reactive, focused, and flexible support to adults following a hospital stay to enable early discharge
- Respond to referrals from Clinicians and Community Health Professionals
- Assess service users' needs, complete and follow a support plan for the individual
- Support service users in the weeks after hospital discharge or to prevent readmission
- Provide transportation to hospital appointments and assist with daily tasks
- Good knowledge of services provided by the NHS and Social Care
- Ability to make things great and improve service quality
- Professionalism and ability to deal with queries in a diplomatic and confidential manner
- Flexibility and ability to work hours outside of the norm
